- PRE-SCREENING QUESTIONAIRE
All clients will receive an email with a Covid-19 self-assessment questionnaire form that must be filled out prior to your appointment. Any additional required consent forms will also be sent prior to your appointment. - Please arrive to your appointment on time. If you arrive more than 5 minutes early, please wait in your car. This will ensure there is only 1 client in the spa at a time and that there is enough time to disinfect the area between clients before each individual appointment.
- Please note that you need to have your face covered with a mask all the time. Your body temperature will be checked upon arrival to the clinic.
- Please wash your hands with soap & water/sanitizer when you enter the clinic and when you leave the clinic.
- All the treatments will be done by appointments only. Only the person getting the treatment is allowed in the clinic. Please refrain from bringing any family/friend with you.
- Once in the clinic, kindly maintain the 2 meter/6 feet social distancing from other patrons.
